Nursing Team

The nursing team are available for:

• Chronic disease management i.e. Asthma/COPD, diabetes, ischaemic heart disease, blood tests, weight and blood pressure checks.
• Injections including childhood vaccinations and travel vaccinations and advice
• Removal of stitches, dressings
• NHS health checks
• Cervical smears

Dementia Annual Review

When you have been diagnosed with dementia, around your birthday month, you will be offered a non-medical annual review.

The Care Coordinator will go through a set of questions with you that will help identify any current support you may need.

Questions are around; your support network, activities of daily living, personal care, social activities, advance care planning, mobility and having a chat about how you are feeling.
